Thank you for your interest in the Eastern Healthcare Preparedness Coalition (EHPC) State Medical Assistance Team (SMAT). Please read the following information carefully as there are several requirements identified that must be completed to receive credit for the training and attain full membership status.
TERMS is a web-based application that you must complete to be a member of SMAT or MRC. This takes about 15-20 minutes to complete. Once on the landing page you will do the following: Existing TERMS User – you can enter your responder information into the “Home / My Account / Response Profile. New Users – Click on “Create Account Now”, under the Have an Account Question. You will need all of your certifications, license, and immunization record. This allows for verification of all information prior to a deployment. You must also complete the paper application so that we can maintain a hard copy of your file for backup. This hard copy also requires that your manager sign off on it so that they are aware of your intentions to participate in the SMAT program.
